Community Partners

 Reflecting its mission, vision and values, St. Joseph's links with other organizations in the community through collaborative partnerships. A range of activity extends from operating joint or coordinated programs, to collaborative planning around specific issues, to participating in health promotion focussed on the community as a whole.

These projects often encompass issues outside of the health-care sector, thus reflecting the notion that "there is more to health than health care."

Community Care Access Centre

 Community Care Access Centres are local organizations established in 1996 by the Ministry of Health and Long-term Care to provide access to government-funded home and community services, and long-term care homes. The South West CCAC is one of 14 in Ontario working together with community partners to enhance access and coordination for people across the province.
