Step 1

Read the General Program Information and Admission Criteria and then identify the most appropriate program for your patient.

Step 2

Complete the referral for admission form for the program.

Step 3

Submit the completed form using the electronic link at the end of the form (with the exception of Long-Term Mechanical Ventilation). Receipt of the form will be acknowledged.

Step 4

The Parkwood Access team registers and reviews the referral.

Step 5

The Parkwood Access team notifies you of the admission decision as soon as it is made.

The goal is to make the admission decision by the next business day. Parkwood Access is open Monday to Friday 8 a.m. to 5 p.m., and closed statuatory holidays.
