Geriatric Rehabilitation Day Hospital (outpatients)
The Geriatric Rehabilitation Day Hospital’s specialized rehabilitation service helps frail patients 60 years of age and older to be healthy, independent and maintain their quality of life. This rehabilitation helps patients who are struggling to maintain their independence: after being discharged from hospital, following a surgery, after an illness, because of declining health.
Admission Criteria for the Geriatric Rehabilitation Day Hospital
- The rehabilitation team involves nursing, physiotherapy, occupational therapy, social work, speech and language pathology, therapeutic recreation, registered dietitian, and psychologist. Medical care is provided by a Geriatrician.
- To be eligible for the Day Hospital program the individual must require 2 or more services.
