Chronic Wound and Skin Health Team
The Chronic Wound and Skin Health Team is made up of voluntary representatives of key players in the prevention, cause and care of wounds. Within Parkwood Hospital this includes: Amputee Program, Adult Seating Program, Nursing, Nutrition, Occupational Therapy, Physician, Physiotherapy, Skin Care Committee, Inpatient Skin Clinic, Continence, and Research. They meet monthly to keep educated, plan workshops, prepare for preceptorships, monitor research activities and look at in-house issues.

The Chronic Wound and Skin Health Team play hosts to numerous Preceptorships throughout the year. For more information, see our educational materials available for purchase.

The Skin Care Committee
The Skin Care Committee is a group of highly motivated and dedicated nursing staff. They meet regularly with the Wound Care nurse clinician to review wound care strategies and current information. They act as a resource to other staff on their respective units and are to be utilized first before a referral is made to the Wound Care clinicians.

Inservices and Workshops for In-house
Staff members/patients of St. Joseph's Health Care, London will have an opportunity to meet and network with the Chronic Wound and Skin Health Team at Parkwood.

In-services/workshops may include:

  • Assessment, treatment and prevention interventions; utilizing a multidisciplinary approach.
  • Challenging issues that are encountered in everyday practice.
  • New practice in evidence-based wound care
  • A discussion of validated tools and the growing complexity of health-care delivery affecting our practice at St. Joseph's Health Care, London.

Inpatient Skin Clinic
The inpatient skin clinic has been operating one half day per month since 1998. The clinic is now located down the hall from the x-ray department and is just outside the outpatient area in room B252.

All of the patients seen in this clinic are in-patients of Parkwood Hospital. The patients who come to this clinic have been referred in-house by other family physicians, or by the nurse clinician for the wound team. The patients suffer various problems from actinic keratoses, suspicious lesions, skin irritations, and chronic wounds. The procedures that are done in clinic by Dr. Keast are as follows: debridement of wounds, application of liquid nitrogen, skin biopsies, removal of moles, and trying to detect the reason for skin irritations.

Inpatient Consults
Inpatient consults can be provided for any in-patient that has complex wounds or requires in-depth wound care assessment in the following way:

  • Unit nurse does initial assessment as part of delivery of care.
  • Unit Resource Nurse (skin care committee member) consulted if further input is required.
  • Wound Nurse contacted if necessary, ext. 44024 or 42163
The Wound Care Nurse will work with the unit nurses to make a holistic assessment and suggestions will be given.

Outpatient Clinic
The Outpatient Chronic Wound Management Clinic began in 1997 and this past year saw 252 new referrals and 918 follow-up visits. This consultative service reaches out to include 17 counties throughout southwestern Ontario. The patients are offered limited follow-up appointments. The clinic offers a holistic approach to wound care for patients who have chronic or complex wounds.

A referral from a physician is required to be seen in this setting. The referral should be faxed to 519-685-4075.

Outpatient Amputee Clinic
The Amputee Clinic is an extension of the in-patient Regional Amputee Rehabilitation Program. The clinic provides discharge follow up from the in -patient program as well as assessing new referrals. The clinic offers a holistic approach to wound care to meet the needs of people who have suffered a partial or total loss of limb(s). Access to the clinic is through a physician referral to fax # 519-685-4075.

Outreach
Workshops provide an opportunity to meet and network with various members of the Chronic Wound and Skin Health Team at Parkwood, as well as other related healthcare providers. Assessment, treatment and prevention strategies may be discussed, utilizing a multidisciplinary approach within the context of the topic addressed. Workshops highlight what is new in evidence-based wound and skin care, and provide for a forum to address clinical experiences.
