Specialized Geriatric Services is a network of interdisciplinary geriatric medicine and geriatric mental health teams. The teams provide clinical consultation, assessment, treatment, education, research and evaluation. A variety of programs and services are offered through inpatient, outreach and outpatient settings.
Programs and services are supported by Geriatricians and Geriatric Psychiatrists from the Divisions of Geriatric Medicine and Geriatric Psychiatry at the Western University.
Teams of health care professionals work together caring for the elderly. The geriatric teams may include:
- Geriatric Psychiatrists
- Social Workers
- Family Physicians
- Registered Dietitians
- Registered Nurses/Registered Practical Nurses
- Occupational Therapists
- Spiritual Care
- Therapeutic Recreation Specialist
- Speech-Language Pathologists
The health care professionals look at the person holistically. The team considers the person’s medical history and present health conditions. The team also looks for the effects of past illnesses, as well as ‘geriatric syndromes’ – common health concerns found in frail elderly such as incontinence, frequent falls, memory problems, and the side effects caused by multiple medication.
Programs and services offered by SGS are located at the following sites and can be accessed through our outpatient intake and triage (Geriatric Ambulatory Access Team) or our inpatient intake (Parkwood Access Office).
