Bundled Care for Elective Shoulder Replacement Surgery

St. Joseph’s Health Care London is providing ‘bundled care’ for elective shoulder replacement surgeries. Through bundled care, all aspects of a patient’s care related to their shoulder surgery and surgical recovery is coordinated across health care providers and settings. This includes care and services provided in the hospital as well as those provided in the community, such as home care and outpatient rehabilitation.

The purpose of bundled care is to help patients having shoulder replacement surgery and their family members/caregivers move smoothly through their journey, from the first pre-operative appointment to the last outpatient physiotherapy appointment, and everything in between.  The goal is to improve the patient/family/caregiver experience and, at the same time, optimize outcomes.
