Your cardiologist or family doctor can refer you to the Cardiac Rehabilitation and Secondary Prevention Program. Once we have received your referral, notification of your orientation session will be sent to you in the mail. This first visit is an opportunity to meet our team, learn about the program and ask any questions you may have. Also at this first visit, you will be given a bloodwork requisition form. Once we have received the blood test results, you will receive notice of your first appointment during which a comprehensive medical assessment will be done to determine the overall health of your heart and your rehabilitation, coaching and counselling needs.
Your family doctor and referring physician will be kept informed of your progress throughout the program.
