Lung Diseases Program: COLD Program (chronic obstructive lung disease)

The Chronic Obstructive Lung Disease (COLD) program offers education, counseling and follow-up for people with chronic obstructive pulmonary disease (COPD). The program helps patients and their families gain a better understanding of the disease.

Improving wellness

The COLD program focuses on self management of COPD following a chronic disease management model and offers an opportunity to increase wellness through education and on-going support. 

A certified chronic respiratory educator (CRE) coordinates care and education for patients, supported by a respirologist and other services offered at St. Joseph’s Health Care, London and community agencies.
