Uploading Diabetes Devices and Sharing Data

Please read the information pertaining to your diabetes device below:

Glucometer

  1. Please record blood sugars and insulin doses using this Blood Glucose Flow Sheet form (Word doc)
  2. Email the completed forms to diabetes@sjhc.london.on.ca and be sure to include your full name, endocrinologist’s name, and appointment date. 

Dexcom sensor

Your endocrinologist's office will email you an invite which wil include instructions to share data with our clinic. 

Please note that your care team will only access your glucose data for your scheduled appointments or at your special request.  

Libre sensor and using the LibreLink phone app to scan your sensor

Please visit libreview.com and enter your LibreLink app username and password. Follow the instructions under Option 2 in this Sharing Libre Reports to share your glucose data with the clinic.

Libre sensor and using the Reader to scan your sensor

Please follow the steps below and refer to the Quick Start Guide for Patients 

  1. Create a Libre View Account. (instructions on page 2 of the Quick Start Guide)
  2. Complete the one-time download for the drivers. (See page 5 for instructions.) 
  3. Link your account with our clinic by going to settings > account settings > my practices > and enter in 07050667 (instructions on page 10).
  4. Upload the Reader by plugging it into the computer using the yellow micro USB cable and follow the instructions on page 7.

Tandem pump

  1. Please follow the steps listed in this Diasend1-pager for Patients document.
  2. Under Step #3, add our clinic ID: 40-63171

Omnipod pump

  1. Please follow the steps listed here: Diasend1-pager for Patients document.
  2. Under Step #3, add our clinic ID: 93-11662.

Medtronic pump

  1. Please follow the instructions in the Carelink Install Guide and the CareLink Personal Uploader.
  2. Please provide your CareLink username and password to your endocrinologist's office.
