South West Frail Senior Strategy event: Co-designing the future state of healthcare for frail seniors in Oxford

Please plan to join us for this interactive and practical day as we work to create a more integrated and accessible system of care for frail older adults in this region.

There is no cost to attend the day and a continental breakfast and lunch will be provided.

Objectives

  1. Discuss and share information about the current state of health care access and delivery, for frail seniors in Oxford.
  2. Reflect on evidence and experience-based future state vision for a South West Frail Senior Strategy, and provide local context to identify priorities, barriers, gaps and opportunities. 
  3. Work together to co-design the future state of health care for frail seniors in Oxford.

Older adults, caregivers, community service providers, and health care professionals in Oxford.
