The Complex Care Program provides compassionate care for patients and families who are dealing with a “life-altering” complex condition resultant from injury or disease. Our interdisciplinary team of professionals works together to support our patients' goals for transitional care and/or to meet their needs for continuing care. Complex Care provides transitional care to support patient needs in anticipation of a return to a community setting (e.g., home or other residential living) when possible; other patients are here to receive palliative and end of life care.
The Veterans Care Program located at Parkwood lnstitute, Main Building and Western Counties Wing provides complex continuing and long term care for Canadian war Veterans. We provide care that assists and supports the Veteran and his/her family to meet their current and anticipated needs to promote health and enhance their sense of well-being and quality of life.
How are we doing?
88.2% of patients/residents responded positively about the quality of care they received in our Complex and Veterans Care programs. 96.0% of families/most significant visitors responded positively about the quality of care their family member/friend received in our Complex and Veterans Care programs. Internally, we set goals related to percent excellent responses and monitor this against the target on an annual basis.
The Complex/Veterans patient/resident percent positive score is the percent of respondents that rated all dimensions positively (e.g., answered “Excellent or Good” when response options are “Excellent, Good, Fair, Poor or Terrible”).
The Complex/Veterans family/most significant visitor percent positive score is the percent of respondents that rated all dimensions positively (e.g., answered “Excellent, Very Good, or Good” when response options are “Excellent, Very Good, Good, Fair, Poor”).
