Community Partner of Distinction Award

The Community Partner of Distinction Award is one of the President’s Awards for St. Joseph’s Health Care London. It recognizes partnership and collaboration - essential requirements of health care delivery and community service. In appreciation of other organizations who demonstrate their commitment to close collaboration through formal or informal partnerships, the Partner of Distinction Award is presented each year to a team or organization that has shown significant support of St. Joseph’s mission and/or the advancement of the health care system in collaboration with others.

This year’s recipient is the Middlesex-London Health Unit’s Outreach Team.

Community Partner of Distinction Award

The Middlesex-London Health Unit (MLHU) Outreach Team is an exemplary community partner of St. Joseph’s. This kind, caring and compassionate group goes above and beyond to support our patients.

In 2016, the MLHU declared an outbreak of HIV andhepatitis C among intravenous drug users in London. Thispopulation is often homeless, marginalized and struggle with addiction andmental health issues. In June 2017, MLHU created an outreach team in response to theoutbreak - the only team of its kindin Ontario. As St. Joseph’s Infectious Diseases Care Program (IDCP) has expanded clinic work within London, the Outreach Team works collaboratively to locate patients, provide transportation toclinicappointments and attends the appointments with patients. Thisteam uses creative and unique ways to connect with patients through social media, email and text messages. In urgent situations where IDCP has needed to reach patients for various health reasons but has no way to contact them directly, the Outreach Team has stepped up to locate, relay a message and support the patient.

St. Joseph's sincerely thanks and appreciates the MLHU Outreach team for their continued support, compassion and collaborative partnership, which truly makes a lasting difference in the lives of patients.

Helping more than 55,000 people in the London region find the right care, in the right place, at the right time, the South West Community Care Access Centre (SW CCAC) plays an integral role in the health and well-being of every segment of the population. The agency is also essential to the work of hospitals and other care providers, and it is this collaboration that has made the agency a standout for St. Joseph’s Health Care London. The agency was recently named the 2010/11 recipient of St. Joseph’s Community Partner of Distinction Award.

“We chose the SW CCAC for this award because they are a longstanding, vital partner of programs and services across St. Joseph’s and the region’s health system as a whole,” says Dr. Gillian Kernaghan, St. Joseph’s president and CEO.

St. Joseph’s and the SW CCAC work together to enable patients to resume community living, provide exceptional care, and support families dealing with the complex health needs of loved ones.

2010 Partner of Distinction Award winner: London Police Service

Compassion and understanding for people with mental illness has won the London Police Service the Partner of Distinction Award, which is presented to a team or organization that has shown significant support of St. Joseph’s mission and/or the advancement of the health care system in collaboration with others.

“St. Joseph’s respects the very close and caring relationship between the police officers in London and health care workers,” says Karen Stone, chief human resources officer at St. Joseph’s.  “Specifically, the London Police Service has helped to reduce the stigma attached to mental health and worked to create effective, compassionate ways to deal with people in crisis.”
