Some conversations we have in life can be difficult; and advance care planning is no exception. Advance Care Planning Day is Thursday, April 16, and the intention of this national day is to help people put some thought into future care choices before those conversations are not possible.
Advance care planning means having meaningful discussions with family, friends or substitute decision makers about your future care choices. It may include writing down wishes and speaking with health, financial and legal professionals.
