How will my appointment be scheduled?

  • You will be contacted by mail or phone with a date and time for your pre surgical screening appointment
  • This appointment will either be a clinic visit or a phone interview depending on your appointment requirements and health status
  • If you are not available for the booked appointment, please call immediately to make other arrangements 519 646-6100 ext. 66016 (Monday to Friday from 9 am to 4:30 pm)

To prepare for your appointment:

  • Plan for a 3 hour appointment
  • Bring a family member or friend
  • Review St. Joseph's Hospital parking information and maps to find the closest parking available in the Cheapside parking garage or non-metered parking 
  • Eat, drink and take all your medications (including insulin) as usual on the day of your appointment
  • Wear comfortable clothes, a button down shirt or blouse, no pantyhose or long underwear
  • Wear your glasses and hearing aids if required
  • Do not wear scented products according to our Perfume and Fragrance Policy
  • If you require an interpreter, please inform your surgeon or staff in advance of your appointment so that one can be arranged
  • If you have a phone interview, plan to conduct the interview in a quiet location free of distractions

What do I bring to my appointment?

  • Ontario Health Card and any additional health insurance information
  • All your current medications in their original containers, including inhalers, medicinal patches, sprays, eye drops, prescription creams, vitamins, minerals and herbal products or recent meds check performed by a pharmacist
  • A list of allergies to drugs, food or environment and the reaction you experience
  • A list of operations, illnesses and health problems including dates
  • Other health related information such as test results, sleep studies, stress test reports etc
  • Money for parking
  • Bring any medication that you may require while at your appointment

What does my clinic visit include?

  • A Registered Nurse (RN) will perform a review of your health history including documentation of all your current medications, and assessment of preoperative labwork required
  • You may have an appointment with a medical consultant and/or an anesthesiologist 
  • A RN will review how to prepare for the day of surgery, which medication to take the day of your surgery and your discharge instructions.
  • If you are being admitted a RN will review what to bring and what to expect while in hospital

What does my phone interview include?

  • A Registered Nurse (RN) will perform a review of your health history including documentation of all your current medications, and assessment of preoperative labwork required
  • A RN will review how to prepare for the day of surgery, which medication to take the day of your surgery and your discharge instructions
  • If you are being admitted a RN will review what to expect while in hospital
  • Please feel free to review your copy of the Guide to Surgery at St. Joseph’s Hospital or contact us if you need a new copy provided
