Rheumatology Clinic: What Patients Can Expect

Each patient who comes to the clinic for the first time fills out a new patient history form including the current medications and dosages that the patient takes.

At follow-up visits a follow-up form is required to be filled out, including current medications.  Patients will also be asked to fill out a Health Assessment Questionnaire (HAQ) at each visit.

These forms help the rheumatologist understand how active the patient’s disease is. To help speed up your visit, please consider downloading and filling out these forms prior to your appointment.

Due to the large volume of patients seen, clinics can sometimes fall behind.  Patients who have flaring of their disease may be added to clinics as required to meet their needs.  We ask for your patience and apologize ahead of time for any inconvenience. We strive to limit delays as much as possible. Waiting times for your appointment may vary so please consider this when choosing your parking. 

If you are unable to attend a clinic visit, please let the office secretary know as soon as possible so another patient can be accommodated in your place.
