Mental Health Care: Concurrent Disorders Services

These services are provided to individuals aged 16 and older living with severe and persistent mental illness and an active substance abuse disorder (meeting criteria for dependence). A consistent care model guarantees the seamless use of psychiatric and substance abuse interventions. All inpatients are screened for the presence of a concurrent disorder. Patients identified as having significant issues can receive consultation and/or services from the ambulatory team. Services include assessment, consultation, intensive care management, tele-psychiatry/shared care and capacity building acorss the Southwestern Ontario region.

Referrals

Referrals to Concurrent Disorders Services can be made by physicians. Length of treatment is determined by individual need.
