Mental Health Care: Concurrent Disorders Services

These services are provided to individuals aged 18 and older living with severe and persistent mental illness and an active substance use disorder. A consistent care model guarantees the seamless use of psychiatric and substance use interventions. All inpatients are screened for the presence of a concurrent disorder. Patients identified as having significant issues can receive consultation and/or services from the ambulatory team. Services include assessment, consultation, intensive care management, tele-psychiatry/shared care and capacity building across the Southwestern Ontario region.


Referrals to Concurrent Disorders Services can be made by physicians through the mental health care electronic referral form, or call 519 455-5110 ext. 47747. Length of treatment is determined by individual need.
