Heart Failure Clinic

The Heart Failure Clinic at St. Joseph’s Hospital is a specialized outpatient cardiac clinic for the care of patients with advanced heart failure.

The goal of the clinic is to optimize quality of life, improve symptom management and prevent hospital admissions. 

With multiple appointments scheduled all the same day, the clinics offers efficient, one-stop care, education and follow-up. Located within the central outpatient area of St. Joseph’s Hospital, the clinic works is in conjunction with St. Joseph’s Cardiac Rehabilitation and Secondary Prevention Program, providing patients with convenient access to various services, such as a range of testing, dietary counselling and exercise programming. 

Taking a multi-disciplinary approach, the Heart Failure Clinic team includes cardiologists, a nephrologist, palliative care specialist, registered nurse, nurse practitioner, pharmacist, and support staff.
