What’s Required for a Referral?

To process a referral, we require the following documents:

  • Letter of referral from a physician or nurse practitioner, confirming follow-up care and prescription renewals
  • Completed intake questionnaires (one from the referring provider, one from the family or caregiver)
  • Psychometric or psychoeducational testing (if available)
  • Proof of involvement with Developmental Services Ontario (DSO) and a signed consent form
  • Behavioral program information (if applicable)
  • Relevant psychiatric and medical reports
  • Any behavioral reports related to the referral
  • Proof of moderate to severe intellectual disability

Referrals will only be reviewed once all required documents are received. It is the responsibility of the referring provider to ensure the referral is complete.

Who Can Refer?

Referrals must be made by a general practitioner or nurse practitioner who agrees to:

  • Remain involved in the patient's care
  • Renew any prescriptions recommended through the consultation
