Elevating Care Quality for Older Adults with Complex Health Needs

Drawing on the expertise of leading geriatric specialists, partner organizations and the lived experience of older adults and caregivers, we are co-designing a more consistent, proactive system of care. One that emphasizes early detection and prevention of disease and disability and promotes sustained well-being for older adults across our region. 

With guidance from Provincial Geriatrics Leadership Ontario, we are committed to leading work in Southwestern Ontario that Supports Clinical Excellence. We accomplish this in the following ways:

Boldly Championing Advocacy

We believe every older adult has a right to access to a health care system that enables them to live their best lives through support from skilled teams, high-quality care and coordinated services. Our advocacy focuses on improving the experience of every older adult and their caregivers. 

Fostering Research and Evaluation

We are steadfast in our commitment to continually improving care and services for older adults through innovative, discovery driven research and ongoing program evaluation that identifies successes and opportunities for growth.

Strategically Leading Capacity Planning

We know access to care when and where it is needed is fundamental for improving health outcomes of older adults. Our success relies on a system that works for the patient and is purposeful in supporting programs that will meet future needs.  

Developing Education and Building Capacity

Every older adult in Southwestern Ontario should have equitable access to expert health care. We’re working with education partners and health care organizations to provide training and education for current and future health care providers in the latest information in diagnosis and treatment of geriatric conditions. We’re also planning ahead and helping determine what health care roles we will need in future to support geriatric care in our region.
