Introducing the Regional Geriatric Program of Southwestern Ontario Strategic Plan 2026–2029

The Regional Geriatric Program of Southwestern Ontario (RGP SWO) is pleased to share its new three year strategic plan: Moving Forward With Purpose. Developed in collaboration with older adults, caregivers, health care partners and community organizations across the region, this plan lays out a shared vision for the future of geriatric care.

At its centre is a commitment to creating communities where older adults and caregivers thrive. The plan outlines how we will strengthen access, enhance quality of care and build stronger, more coordinated supports across all eight sub regions of Southwestern Ontario.

Through three clear strategic directions—Leading Excellence Across Every Community, Empowering Equity Through Access and Driving Timely Care—the plan provides a roadmap for meaningful, measurable improvements that reflect the voices and needs of the people we serve.
