St. Joseph’s Aging Brain and Memory Clinic provides assessment, diagnosis and treatment for people with memory issues. We also care for people with cognitive impairment. Cognitive impairment refers to a decline or change in the brains ability to function. There are several different stages of cognitive decline. Some changes are a natural part of aging. Others are caused by diseases and conditions such as dementia. To access the Aging Brain and Memory Clinic, patients must have a referral from their primary care provider sent through the Geriatric Ambulatory Access Team.
South West Nurse-Led Outreach Team (NLOT) partners with long-term care homes to improve residents’ access to specialized care. The South West NLOT team at St. Joseph’s Health Care London (St. Joseph’s) supports homes in Elgin, Grey Bruce, Huron Perth, Middlesex-London and Oxford. Our team includes nurse practitioners (NPs) and registered nurses (RNs) who specialize in caring for older adults.
