South West Nurse-Led Outreach Team (NLOT)

South West Nurse-Led Outreach Team (NLOT) partners with long-term care homes to improve residents’ access to specialized care. The South West NLOT team at St. Joseph’s Health Care London (St. Joseph’s) supports homes in Elgin, Grey Bruce, Huron Perth, Middlesex-London and Oxford.

Our team includes nurse practitioners (NPs) and registered nurses (RNs) who specialize in caring for older adults. South West NLOT is operated by St. Joseph’s and supported in partnership with the Regional Geriatric Program of Southwestern Ontario.
