The South West Nurse-Led Outreach Team (NLOT) supports long-term care homes by providing specialized care and resources tailored to the needs of residents, caregivers, and staff.

South West NLOT supports long-term care homes in providing care to residents within the home, helping to avoid hospital visits whenever possible. Long-term care home staff can contact South West NLOT for support as needed. When involved in resident care, South West NLOT nurses will engage with residents and their caregivers and with home staff and physicians to plan and deliver care.
