Outpatient Rehabilitation
In the interest of reintegrating and maintaining individuals with an acquired brain injury in their own community, this program offers hospital clinic and/or community based rehabilitation services.
The Brain Injury Outpatient Program provides treatment, support, training, and education to people with acquired brain injury ("clients"), families and caregivers. The program is Ministry of Health funded and is available at no cost. Referrals for in-clinic therapy are accepted from the 10 counties of Southwestern Ontario; community-based services are limited to London and surrounding area.
What Services Do We Provide?
The Acquired Brain Injury Outpatient Program provides client-focused treatment that involves clients, families and caregivers as rehabilitation partners. Services include but are not limited to:
- initial assessment and ongoing support by a physiatrist (physician specializing in physical medicine and rehabilitation)
- discipline-specific assessment and therapies, including occupational therapy, physiotherapy, psychology, speech-language pathology, social work, and/or therapeutic recreation
- community-based rehabilitation provided by a rehabilitation therapist, under the direction of a regulated health professional
- cognitive rehabilitation
- communication skills development
- psychosocial counselling
- training and support with activities of daily living
- vocational and academic rehabilitation
- recreational/leisure skill development
- community reintegration
- counselling and education about acquired brain injury, including a family and survivor education series
- vision rehabilitation clinic
- return to school program
The Return to School (RTS) treatment-based program is giving young adult students with mild brain injuries such as concussions the skills to successfully return to high school, college or university. Students participating in the RTS group are engaged in cognitive-communication assessment/screening with a speech-language pathologist prior to attending the group to identify specific areas of focus.
What Can You Expect?
- a determination of which services are appropriate, based on individual client assessment, and in collaboration with the client/family and other relevant service providers
- client-centred support and advocacy
- evidence-based practice
- individual rehabilitation plans completed prior to treatment, which outlines specific treatment goals as well as timelines for their achievement
- timely documentation which reviews progress towards treatment goals
- regular communication between the treatment team, the client/family, other service providers
A physician's referral is required (download the Acquired Brain Injury program referral form). Referrals are then triaged and made to other team members for discipline-specific assessment and treatment as appropriate.
