Primary Care Diabetes Support Program

The Primary Care Diabetes Support Program is located in the St Joseph’s Family Medical and Dental Centre at 346 Platts Lane in London, Ontario and provides comprehensive and multidisciplinary diabetes support and management to individuals with Type 2 diabetes.  

We specialize in the following:

  • Providing diabetes management to individuals who do not have a primary care provider.
  • Supporting individuals with various challenges can make managing diabetes more difficult, such as; economic hardship, language barriers, and any issues or illnesses that can make navigating the health care system more difficult.  
  • Delivery of innovative group programs  
  • Delivery of specialized diabetes foot ulcer prevention and management  

Our multidisciplinary team consists of Diabetes Canada certified Registered Dietitians, Registered Nurses, Nurse Practitioners, a Social Support Worker and Family Physicians specialized in diabetology.  

We provide innovative care that includes in-depth clinic visits, workshops and expertise to assist individuals in incorporating lifestyle as medicine (nutrition and activity), and close telephone and email follow up to help with blood sugar control if needed.  Opportunities to participate in clinical trials and receive assistance from a social support worker when required are also possible.
