Uploading Diabetes Devices and Sharing Data
Please read the information pertaining to your diabetes device below:
Glucometer
- Please record blood sugars and insulin doses using this Blood Glucose Flow Sheet form (Word doc).
- Email the completed forms to @email and be sure to include your full name, endocrinologist’s name, and appointment date.
Dexcom G7 sensor
- Open your Dexcom G7 app on your phone
- Tap the Connections Tab
- Tap Clarity Clinic and tap “Continue”
- Enter the Clarity Clinic Code when prompted: sjhclondon
- Confirm clinic details and tap “Done” when finished

Dexcom G6 sensor
- Open the Dexcom Clarity app on your phone
- Tap on “Profile” in bottom right corner
- Tap on “Manage Data Sharing”
- Enter the Clarity Clinic Code: sjhclondon and tap “Continue”
- Confirm clinic details and touch “Confirm” when finished

Libre sensor and using the LibreLink phone app to scan your sensor
In the FreeStyle LibreLink app, go to Menu > Share > LibreView > Connect to a Practice > Enter the Practice ID 07050667 and click Next then Connect.

Libre sensor and using the Reader to scan your sensor
Please follow the steps below and refer to the Quick Start Guide for Patients
- Create a Libre View Account. (instructions on page 2 of the Quick Start Guide)
- Complete the one-time download for the drivers. (See page 5 for instructions.)
- Link your account with our clinic by going to settings > account settings > my practices > and enter in 07050667 (instructions on page 10).
- Upload the Reader by plugging it into the computer using the yellow micro USB cable and follow the instructions on page 7.
Omnipod and Tandem pumps
Tandem pump
- Visit source.tandemdiabetes.com and log in to your account
- Click the Profile icon in the top right corner of Tandem Source
- Click on the blue icon at the top right to access Account Settings
- Click Share Reports on the left side of the Account Settings screen
- Search for clinic and identify correct address then select it
- Click Start Sharing. Tandem Source returns to the Account Settings screen and displays a success banner at the top of the screen
Omnipod DASH pump
Please follow the steps listed on Glooko webpage for creating an account:
- How do I create and access my Glooko account? – visit Glooko Support | Help Center
- Enter the ProConnect Code: sjhclondon when prompted for your code
- To upload your pump, refer to How do I upload diabetes data on the web using the Glooko® Uploader (Personal)

Omnipod 5 pump
- Log into your online Omnipod® account at www.omnipod.com using your Omnipod ID
- Go to the My Account tab and then select Account Linking from the drop down menu
- Select sign up or log in with Glooko
- Proceed to Glooko and complete the process to link your account. When complete, your status will update to Linked and data will automatically upload into your personal Glooko account

Medtronic 670G pump
Please follow the instructions in the Carelink Install Guide and the CareLink Personal Uploader. We will connect your account to our clinic at your appointment.
Medtronic 770G and 780G pump using the CareLink uploader with the blue adapter
Follow the instructions in the CareLink upload guide for creating your CareLink personal account and uploading your pump to your computer.
Medtronic 770G and 780G pump using the MiniMed Mobile app
Follow the instructions in the MiniMed mobile app sharing guide for setting up and using the MiniMed Mobile app.
