Specialized Geriatric Services: Geriatric Rehabilitation Day Hospital - What To Expect

At the first appointment, a nurse talks to the patient about health care challenges they’re having and determines how the Geriatric Rehabilitation Day Hospital can meet their needs, such as becoming stronger to carry out daily tasks, learning memory tricks to remember appointments, or preparing easy, nutritious meals.

The next assessments occur during the second visit. The patient is seen by the right health care professional(s) who can help meet their needs. This may be a geriatrician, and/or occupational therapist, and/or physiotherapist and a social worker. After this visit an individualized care plan is developed for the patient.

Then the regular program visits begin which last from six to eight weeks depending on the treatment goals.
